My Pamper Party - Booking Form

My Pamper Party will plan your wonderful Pampering Day Spa Party for you!

Booking Instructions:

  • A £35.00 non refundable deposit is required to secure the party date by Credit/Debit Card, PayPal or Cheque at least 4 weeks prior to the Pamper Party
  • Balance will become due in full 14 days before party date
  • Please take time to complete the booking form in full with all the details required. This will enable us to plan your treatment schedule and make the pamper party run smoothly
  • List names of guests and treatments required. For under 18's please state age
  • Celebrating a special occasions let us know the special occasion and name of Hen, Birthday girl etc
  • Before submitting your booking form please pay your deposit first and then click submit
  • Or if you prefer to pay by cheque please print out the completed booking form and post with the cheque to address listed below
  • On completion of the PayPal transaction you will be sent a PayPal receipt by email. Please make sure you have your correct email address and details on your PayPal account form.
  • If you don't have a PayPal account you can easily open one when following the payment process or select the option "If you do not currently have a PayPal account click here button" to pay by credit or debit card within PayPal. PayPal is a secure method of payment and used by millions of people around the world for fast and secure transactions.
